Additional notes on written records:
A major focus of this clerkship is appropriate communication between health care providers. Inpatient H & Ps and outpatient SOAP notes, as well as oral case presentations, provide the opportunity to review this communication. Please insure that all such communication is done in a professional manner. Handwritten write-ups are acceptable if neat and readable. Dictations are not write-ups and are not acceptable. If you choose to use a write-up template, delete all inappropriate or unasked questions.
